In order for DSL service to work with the other telephone devices in your home, it is necessary to place a microfilter on each telephone device being used in your home.

In order to send the correct amount of microfilters, it is important that we know what equipment is connected to the line that will receive Qwest DSL service. Please enter the correct numbers below:

How many wall mounted telephones are currently being used in your home*?
How many other telephone devices* including desk telephones, answering machines, FAX machines, computer modems, utility monitoring devices, DSS satellite receivers, and burglar alarms are currently being used in your home?
*Additional charges may apply if you have more than 1 wall mounted telephones or more than 3 other telephone devices. Based on the numbers you enter above, USFamily.Net will ship the number of wall mount and desk micro filters that you need and have requested. Each modem comes with 1 wall mount and 3 desk micro filters included at no additional charge. Price for each additional desk microfilter is $5.00. Price for each additional wall microfilter is $7.50
